Helping people rebuild their lives
NCHA’s Mental Health Community Rehab team is supporting people across Nottinghamshire to rebuild their lives after leaving inpatient mental health care.
Working alongside NHS partners as part of a multi‑agency team, the service – commissioned by Nottingham NHS Trust - provides personalised support to people moving from hospital or residential rehabilitation back into the community. The aim is to reduce readmissions and support long‑term recovery.
Support usually lasts around 12 weeks, but can be extended for up to two years where needed. Working one‑to‑one, support workers help people rebuild routines, access services, and regain confidence and independence. Since launching in September 2024, (and as of April 2026) NCHA has supported 139 people through the service, supporting positive outcomes.
Real impact
The impact of this approach can be seen in the progress people make.
One person supported by the team had been experiencing severe isolation and low confidence. With support, he began attending local groups, rebuilding social connections and learning to travel independently – even visiting family on his own for the first time in a long time.
For others, progress starts with small steps. One service user initially found it difficult to leave home. With consistent support, she moved from home visits to meeting in local cafés, and now spends time independently in the community and uses her phone again – something that had previously caused distress.
Supporting recovery at the right time
Support often begins before discharge, providing reassurance at a critical point and helping people transition more smoothly into the community.
Service manager Fran said: “This service is about stepping in at the right time and giving people the support they need to move forward. By working closely with our partners, we’re helping people stay well, stay independent and build more positive futures.”
At its heart, this service is about people – making sure no one leaves hospital feeling alone or unsupported. Whether it’s taking those first steps outside, rebuilding confidence, or finding purpose again, the team is there every step of the way.
And it’s those small, steady steps that lead to lasting change – helping people not just return home, but truly move forward with their lives.
