How we can help
We are a team of 12 Mental Health trained support workers focused on enabling citizens to live independently in their community.
Based only within the Nottingham City Council boundaries delivering support that is time limited up to 12 weeks, directly addressing issues that may be negatively impacting on your wellbeing.
Working closely with Nottingham City Adult Social Care the pathway and first point of call for anyone who needs support, they provide our referrals based on the needs of the individual. Offering support to anyone over the age of 18 with varying levels of needs, we actively try to overcome barriers that can affect daily life.
Over the year of 2025 we have supported 217 citizens to achieve recovery and improve their lives.
As a team we have promoted a diverse range of citizens to have greater control over their lives and safeguarded individuals in a period of crisis. We operate under a passionately, holistically and practical approach.
So how does it all work?
Most of our referrals come from the Nottingham City Health and Care Point, an accessible telephone or online service that anyone can use.
This is where Adult Social Care will direct you to the right support service, in most cases they will offer Mental Health Reablement so that we can support and identify any further support needs.
Once you have been offered and accepted a referral to us our referral coordinator will be in contact, they will answer questions you may have and advise on our current wait list times for a support worker.
When you have been allocated a support worker, they will contact to introduce themselves and book in a ‘needs assessment’.
We have many ways of making this a smooth process but it gives our support workers an opportunity to get to know you and identify exactly how we can help.
After this is completed and you have agreed with your support worker your most needed support areas, we can commence support. W
here possible we would like to support you face-to-face on a weekly basis, we can accommodate for your preference.
