Mental health community rehab
We have a suite of services across the East Midlands for people experiencing mental ill-health, supporting them and aiding recovery.
Most recently, we’ve been commissioned by Nottinghamshire Integrated Care Board and the Nottinghamshire NHS Trust to support people transition from inpatient settings, back into the community, and longer-term rehabilitation where needed.
The Mental Health Community Rehab Team work across Nottinghamshire County and Nottingham City, as part of the NHS Community Rehabilitation Team. Together they are supporting people in their mental health rehabilitation journey to long term recovery away from hospital care.
Establishing a routine and gaining trust
Tracey Clarke, Support Worker talks us through a recent referral:
"I recently supported someone who had been in and out of hospital a number of times in the last few months.
"They’d always struggled with mental health, but work and family pressures had brought them to a point where their mental health was impacting their relationships, work life and they were having suicidal thoughts.
"After their most recent hospital admissions, they were referred to our service to help them settle in back at home and reduce the chance of a relapse.
"Being at home was hard for them. They experienced bouts of loneliness and struggled to fill their time.
"I spent 12 weeks supporting them to find meaningful and productive activities that helped improve their mood and support them on the road to recovery.
"We definitely had ups and downs, but slowly they started to trust me. We started to explore group sessions and activities and went on lots of walks together. We spent lots of time over coffee talking about what they’re like to do when they felt better.
"Group sessions were hard for them to begin with, but I saw their confidence blossom and begin to open up to others. Eventually they felt able to attend these sessions without my support.
"Together we established a routine that included daily walks and contact with friends, and a weekly plan of activities gave them things to look forward to.
"The 12 weeks that we spent working together, was the longest time they’d been out of hospital for a very long time. They were still experiencing spells of low mood, but they had the tools and knowledge to help them cope better and live a life full of positive experiences.
"It was an absolute pleasure to see the transformation in their confidence, and support them on the road to recovery."
