How we can help

We provides up to two years of low-level, housing-related support for people needing help to keep and maintain their home. We provide advice, support and assistance to develop independent living skills and maximise independence.

Everyone at the service has a named support worker who helps to create a person-centred support plan for your goals. These plans are regularly reviewed to ensure we are meeting your needs and helping you to develop.

Support workers keep in touch via phone, text or email and through face-to-face contact in the community. The length of support people receive will depend on their individual needs. Support is available for up to two years, but many people achieve their goals before this time.

The service is funded by Derbyshire County Council and delivered by us.
