Transforming lives with Transforming Care
Our Transforming Care services are shortlisted for this afternoon’s UK Housing awards in the Best Supported Housing Landlord category.
We couldn’t be prouder that our teams are being acknowledged in this way.
NCHA have eight Transforming Care teams, offering 64 tenancies. These schemes support NHS England’s Transforming Care targets, to provide long-term, sustainable community-based services as an alternative to long-term hospitalisation for adults with complex learning difficulties, autistic spectrum disorder, associated mental health conditions and behaviours of concern.
Many of our Transforming Care residents have come to us following long spells in hospitalised environments. At our schemes they are encouraged to live fulfilled lives, increase independence and make new friends. Each residents has a personalised support plan that is predominantly about supporting independent choice, including social activities and meal plans.
Each Transforming Care resident has their own independent home on a secure tenancy with front door, kitchen and lounge along with a bespoke package of support. The Specialist Housing Team help them sustain their tenancy and Support Workers assist them to manage their finances and look after their homes.
We have seen incredible transformations as individuals learn to live in a community-based setting, and gain independent choice in the way they want to live their lives and plan for the future.
We have developed close relationships with local authorities, commissioners, health professionals and Transforming Care Partnerships to deliver accommodation and support with such positive results.
Our Transforming Care teams are quite simple transforming people’s lives.
